Having worked in industry including robotics system development I would suggest that it is important to consider new technologies, redundancies and statistical probabilities on this issue closely.  The focus needs to be improving safety results in regards to hazardous energy incident prevention and should consider practical operations as well as human behavior considerations. No serious discussion should be conducted without honest detailed statistical analysis of incidents over the last 20 years as technology has evolved.
